This disclaimer applies to the Men of Eros website, operated by Form V Creative, LLC, doing business as Men of Eros and Form V Productions. It supplements any applicable ticket, venue, booking, admission, or event agreement. A separate written agreement controls if it contains different terms.
Age restriction
The website and its content are intended only for adults age 18 and older. Individual venues or events may require guests to be 21 or older or may impose other admission restrictions. Guests are responsible for confirming and complying with the rules for their specific event.
Adult live entertainment
Men of Eros provides adult-oriented live entertainment. Events may include loud music, theatrical lighting, crowds, suggestive themes, audience participation, and physical proximity to performers. Participation is voluntary. Guests should use their own judgment, follow venue and performer instructions, and refrain from participation if it may be unsafe or inappropriate for them.
Independent performers and third parties
Independent contractors, including performers and other event-service providers, may be engaged under separate agreements when consistent with applicable law and the actual working relationship. Men of Eros does not control independently operated venues, ticketing services, customers, or the acts of third parties outside its reasonable direction and control.
Risk and responsibility
Events involve ordinary risks associated with live entertainment, crowds, venues, travel, and voluntary audience participation. To the fullest extent permitted by law, Form V Creative, LLC and its DBAs, Men of Eros and Form V Productions, are not responsible for accidents, injuries, property damage, unlawful conduct, or other losses caused solely by the acts or omissions of an independently operated venue, customer, ticketing provider, or independent contractor, including a performer or other event-service provider, acting outside the scope of an applicable agreement or outside Men of Eros' reasonable control.

Performer lineups
Performer lineups vary by event and are subject to change based on availability, illness, travel, scheduling, or other operational needs. Performers shown on this website or in promotional materials are not guaranteed to appear at a particular event unless expressly confirmed in writing.
Tickets, event changes, and refunds
Ticket sales are final and non-refundable unless the terms presented at purchase provide otherwise or applicable law requires a remedy. Events, locations, schedules, and performer lineups may change. If an event is postponed, relocated, rescheduled, or canceled, any available credit, exchange, or refund will be governed by the applicable ticketing, venue, promoter, or event terms and non-waivable law.
No guaranteed results
Promotional descriptions do not guarantee ticket sales, attendance, revenue, event outcome, a particular performer, or suitability for a particular audience. Confirmed obligations are limited to those stated in an applicable written booking or ticket agreement.
Acceptance in transactions and admission
This website disclaimer governs use of the website. For a ticket purchase, booking, or event admission, it becomes part of the transaction only when it is presented or incorporated into the applicable checkout terms, booking agreement, ticket terms, or admission rules. Where it is incorporated, purchasing a ticket, signing or accepting the booking agreement, or entering the event constitutes acknowledgment of the incorporated terms.
Emergency and safety matters
Guests and event organizers should immediately report unsafe conditions, injuries, or emergencies to venue personnel and contact emergency services when appropriate. This website does not provide medical, safety, or legal advice.
